New Patient Registration
We welcome new patients to our Practice, providing you live in our Practice Area. From 5th January 2015 GP practices were offered the opportunity to register patients who live outside their practice area without any home visiting and urgent care obligations. This policy was not mandatory and therefore GPs were given a choice as to whether to participate or not.
After due consideration this practice had decided NOT to accept out of area registrations.
If patients wish to review the documentation associated with this policy they can request guidance from NHS England. This is in the form of a booklet entitled "Choice of GP Practice - Guidance on the New Out of Area Patient Registration Arrangements" (Published December 2014).
How to Register
The registration process is simple. Please call into the surgery to obtain a new patient registration form. You will also be asked to complete a health status form. This will help us to treat you if you need attention before your medical records arrive from your previous GP.
If you are not an existing NHS patient we have to verify that you are entitled to free NHS treatment. This will require proof that you are entitled to remain in the UK e.g. Visa, entry clearance. Persons from EU countries, or returning UK Nationals wishing to reside, will be asked for proof relating to employment or residency, as directed by the Department of Health.

New Patient Health Checks
When you register with this practice you will be offered an appointment with the Practice Nurse for a health check. This will give us an indication of your health needs prior to us receiving the notes from your previous GP. Please try to attend this appointment.
Also, if you are 75 or over and have not been seen by a health professional within the last 12 months you may request a health check by our Practice Nurse. Please contact the surgery to make this appointment.
