Before filling out a registration form, check you are in our catchment area.
Because this is a rural area the postcode locater is not always accurate. If your address is located on or near the border please contact the surgery to ensure you are located within the boundary. This will avoid further disruption at a later date.
You can also find out which GPs you can register with on the NHS website.
Registering with the surgery
To register with the surgery:
- use the NHS Register with a GP surgery: Rainford Health Centre service
- come into the practice for a physical registration form
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
